It is widely assumed that respiratory-muscle weak-ness, either intrinsic or due to fatigue, leads to respiratory failure.1 For the past 30 years, atten-tion has been focused on whether the diaphragm muscle is especially susceptible to fatigue in pa-tients with chronic pulmonary disease and criti-cal illness, increasing the potential for respiratory failure.
